State the unresolved question

Identify the asset or service program, the current status and the action that needs approval or specialist input. Summarize authorized work already completed and the evidence available. Separate confirmed facts from assumptions. Describe the operational coordination needed without inventing a technical severity or a response commitment outside the actual service arrangement.

Name what the answer will release

Explain which visit, part order, configuration review or communication depends on the decision. Give the reviewer a contact who can answer follow-up questions. Record the decision in the main service record and tell the affected teams how it changes their next step. If the answer creates a new dependency, assign that item explicitly rather than assuming the escalation itself closed the request.
